We help children throughout the U.S. who are under the age of 21 and have medically related hair loss. Our recipients have different hair colors and textures, and we make every effort to provide hair replacements that are similar to their natural hair.
Please read through the requirements below BEFORE starting the Hair Application Form.
The applicant must be a United States Citizen or have a United States Permanent Resident Visa Status. The applicant must reside in the United States. A Birth Certificate of the applicant is required. A Driver’s License or State ID of the applicant’s parent/guardian is required.
A letter from the doctor must be on their letterhead, signed, dated, and include the applicant’s name, birthdate, hair loss diagnosis, and the date last seen by the doctor.
Before hair loss photos and current photos showing hair loss are required. These photos are used to help choose a wig or hairpiece that closely matches the hair that was lost and/or the best fit for the type of hair loss diagnosis. These photos are never published.
