We help children under the age of 21 with medically related hair loss. Our recipients are both boys and girls of all ages under 21 years old, with different hair colors and textures. We make every effort to provide a hair replacement that is very similar to your natural hair color and texture!
Please read through the requirements BEFORE starting the Hair Application Form below.
The applicant must be a United States Citizen or have a United States Permanent Resident Visa Status. The applicant must reside in the United States. A Birth Certificate of the applicant is required. A Driver’s License or State ID of the applicant’s parent/guardian is required.
A letter from the doctor must be on their letterhead, signed, dated, and include the applicant’s name, birthdate, hair loss diagnosis, and the date last seen by the doctor.
Before hair loss photos and current photos showing hair loss are required. These photos are used to help choose a wig or hairpiece that closely matches the hair that was lost and/or the best fit for the type of hair loss diagnosis. These photos are never published.
